Making Changes / Qualifying Life Event Checklist

Adding Coverage –

Please see the above document labeled “Making Changes / Qualifying Life Event Checklist” under the Adding Coverage section that explains the qualified event process and the documents you will need to provide so we can submit the request to add coverage through the district due to gaining other coverage(s). Please provide this information immediately as this information is time sensitive.  Please note: You only have 31 days from the date of the event to make this change.

There are two (2) forms that must be completed, signed and dated. 

  1.  TRS Aetna Enrollment Application and Change Form (2 pages)
  2.  HR Qualifying Event Change Form (1 page)

Please ensure that you send all information, including your proof of the Qualifying Event directly to me at Cynthia.Botello2@hayscisd.net so that your request is processed accordingly and timely.​

​Benefit plan information and rates can be located under the appropriate benefit link under the Benefits Tab at: http://www.hayscisdbenefits.com/benefitsite/.​

Note:

Documents must be submitted within 31 calendar days from the event date. Premiums are deducted one month in advance (August payroll pays for September premiums and so forth). There may be a possibility of multiple premiums owed based on the event date and when completed documents are returned to our office. It may take up to two pay cycles to see an adjustment on your pay check.

 

Dropping Coverage –

Please see the above document labeled “Making Changes/Qualifying Events Enrollment Information” under the Dropping Coverage section that explains the qualified event process and the documents you will need to provide so we can submit the request to drop coverage through the district due to gaining other coverage(s). Please provide this information immediately as this information is time sensitive.  Please note: You only have 31 days from the date of the event to make this change.
